正確的餐廳"付帳"禮儀? - 文化差異

不好意思,我有查過這方面的資料,
但是大家對於這方面的講法都不太一樣,
也可能是因為每個國家的民情不同,但是我還是想聽聽各位的經驗談。
我想請問的是,像去歐美那些餐廳用餐,
通常結帳的時候,應該怎樣的"流程"比較正確?
我知道這個問題有點奇怪,但是我真的有點困擾...XD
表明要結帳,拿到了帳單之後,
如果是用信用卡結,我就會將卡片跟帳單夾在一起,
等負責我那桌的服務生來收走,然後等他刷好卡片,拿給我簽名,完成!
第一個問題:
如果發現這家餐廳沒有 tips included / service fees included,
此時,小費要怎麼給?寫在帳單上?直接寫 xx% ?
如果服務生多輸入小費的金額....怎麼辦?
第二個問題:
如果我是要用現金結帳,而且我趕時間,我等不及服務生來收,
我可以將現金(包含小費)直接留在桌上,然後走人嗎?
還是要直接拿去給服務生?
第三個問題:
一般小費是多少 % 比較合理?
到底怎樣作才比較不會失禮?
